The best thing to do in a situation like this is to think about how things are now, and how they used to be. Figure out how things have changed in good ways or bad ways. Talk to him about it, tell him how you feel. From the sound of it, he sounds super deep and sensitive, obviously he would get what it is that you feel.

He chose to take a different path in life; going in the opposite direction than yours and because of that his whole perspective concerning you has changed. It has nothing personally to do with the way you look, only with the way he views you. I'm sure you're as beautiful as always.
He has CHANGED, not the way you look, only the way he views you because of the change in him.
